Targeted Case Management is designed to meet the needs of an individual by providing referrals and linkages to services like housing, transportation, insurance, Medicaid benefits, transitional care and follow-up, and more.
Psychosocial Rehabilitation aims to build or restore skills that will assist an individual with successful community involvement while reinforcing clinic skills. These skills may include daily living skills, wellness skills, maintaining relationships with friends, family, or providers, gaining employment while maintaining benefits, and more.
Family Peer Support Services provides support to caregivers of children with a Mental Health Diagnosis. They provide role modeling, parenting skills, and strategies to cope with challenges. They can help with parental skill development, educational support, transitional services, acquiring general resources for basic needs (SSI/SSDI, DSS, employment) and more.
Adult Peer Support Services offers support to adults who are living with Mental Health and Substance Use challenges. Support can include developing appropriate coping skills, healthy and positive lifestyle modeling, and more.
